Online Services

This practice currently offers the following online services to patients:

• Book, view, amend, cancel and print appointments online.
• Order, view and print a list of repeat prescriptions for medications or appliances
• Order, view and print a list of acute prescriptions for medications or appliances (issued within the last 6 months)
• View, export or print summary information from record, relating to medications, allergies and adverse reactions.
• View detailed coded information held within medical record
• Full Prospective Record Access (access to information and data added to the patient record from a set date onwards)

If you would like to request online access you can do so by printing the relevant form below, complete it and bring it into the practice along with a form of ID, ideally photographic, e.g. Passport or Driving Licence. If you do not have one of these you can discuss alternatives with the receptionist when you attend with the completed forms.

If you have completed an application form for record access please do not also request via online services as duplicate requests may hold up processing your request.

The Practice has the right to remove online access to services for anyone that doesn’t use them responsibly
