New Patients

When you register you will be registered with the Practice as a non limited partnership. We will offer you a named GP partner on registration, and you will be offered the GP of your choice whenever possible.

All new patients when registering with the Practice will be advised of their ‘named GP’.

The GP we are currently registering new patients with will be displayed at the front reception desk.

You may also ask for this information at Reception when visiting the practice.

Please also complete and return the New pt Med.Questionnaire  If you are registering a newborn baby please complete and return the Newborn Health Questionnaire

This information is important to your new GP when you register.

If you are registering a child aged under 5 years, please complete the Health Visitor Registration form, which you can download here and return it to the surgery with the registration forms.

Once all necessary forms are completed please present these to reception in person.

In order to allow time to check your forms and answer any queries you may have we ask if possible to avoid early morning or end of surgery ideally between 10am and 5pm.

You will be required to show two forms of identification; one should be photographic. This can be a passport or driving licence and the second must be proof of your current address. This could be a utility bill, tenancy agreement or bank statement. If you are struggling to provide a form of identification please discuss this with reception.

Once we have received your forms please allow up to ten working days to process your registration. After this period you will be fully registered unless contacted by a member of staff.

We encourage our patients to try to see one or two regular GPs for any ongoing medical condition to maintain continuity. This need not be the GP with whom you are registered.
